Finish grilles need clean clearance and careful fastening. Pull the frame, true the opening, repair finish damage, and reset without forcing the trim.

Plain-English answer
Interior door grilles are part mechanical and part finish work. If the opening is too tight, hammering the aluminum frame can splinter veneer or bend the trim where everyone can see it.
Remove the grille, clean the rough opening with the right finish tool, and reset with even clearance. Repair damaged veneer/paint per the GC finish direction before tightening the sleeve screws.
